You must agree to the following by checking the box below each statement.

1. "At the time of registration, I agreed to a four-month minimum. If I have not met the minimum, I will pay any remaining fees at the time of cancellation. I also understand that I am responsible for payment of any past due balances."

2. "I understand that if I reinstate my membership at a later date, my rates will be increased to the current monthly dues."

3. "I understand it is my responsibility to ensure that all billing has stopped. I will contact the Aztec Recreation Center within 30 days to resolve billing problems. Refunds will not exceed 90 days w/o confirmation."

4. "I understand this form must be received before the 24th of the month to avoid further billing."

5. "I will retain my email confirmation letter as receipt of cancellation. I understand refunds will not exceed 3 months unless I have my copy of confirmed cancellation."
