Member Cancellation Request


To cancel your membership at the Aztec Recreation Center, please complete the form below. If the following requirements have been met your cancellation will be processed this month:

  1. Request submitted on or before the 24th of the prior month I wish to cancel.
  2. Met 4 month minimum agreement.
  3. No outstanding balance on account.

You must agree to the following by checking the box below each statement.

1. "At the time of registration, I agreed to a four-month minimum. If I have not met the minimum, I will pay any remaining fees at the time of cancellation. I also understand that I am responsible for payment of any past due balances."

2. "I understand that if I reinstate my membership at a later date, my rates will be increased to the current monthly dues."

3. "I understand it is my responsibility to ensure that all billing has stopped. I will contact the Aztec Recreation Center within 30 days to resolve billing problems. Refunds will not exceed 90 days w/o confirmation."

4. "I understand this form must be received before the 24th of the prior month to avoid further billing."
